Who or what was Book before he became a preacher? What did the Alliance officer see on Book's ident card?

Can anyone tell what Mal says to Simon during the rescue? Mal starts, "Gotta say, doctor." and then Simon responds "Yes, I'm very proud." What does Mal "gotta say"? Something about "the town" and "miraculous", perhaps?

When Serenity's registration papers are shown to the Alliance officer, he addresses Mal as something like Karbapkin? Does anyone know for sure who Serenity is registered to or has a script of this episode?

For those wondering how Book's ident card got him special attention, I speculate that Book used to be an Alliance colonel or general prior to retirement and becoming a shepard. It explains why he knows so much about ships in the Serenity episode. It might also explain how Book knows about the Fed who hold sthe crew at gunpoint in the cargo bay. It also fits how he is pretty good in a fight and understood how to use firearms in War Stories.
For those who saw the movie, I suspect Book also knew about the secret because he's ex-alliance...but couldn't tell Mal (I won't spoil it for everyone, go see the movie)

My question is this: if Book really was some sort of former Alliance member of any sort, especially in the military sector, why then at the end of the pilot is he on the verge of tears confessing to Inara his fears of falling in with criminals and being shot at?

Maybe law enforcement? His only qualms seem to be about being involved with illegal or unsavory activities...

good call on that muffled line from mal! if you turn the subtitles on in the DVD, it just says "indistinct" or something like that! :)

On the UK version of the DVD, the subtitles for that "indstinct" line read: "The town is real and the people near miraculous." I always thought that didn't make any sense whatsoever!

According to the shooting script, Mal's garbled line at the end that even the DVD subtitles don't know is, "your talent for alienating folks is near miraculous," to which Simon replies, "Yes I'm very proud."
