The first time she got it was after Faith threw it at one of the spider things the Mayor was going to eat in "Choices" and Buffy confiscates it from there.

The second time- After Faith falls from the roof in Grad Day P1 you see that Buffy still has the knife but in P2 you see her leave it on the roof. Later on in that same episode when Buffy is planning the distraction for the mayor she say something about needing to pick something up first- referring to the knife. We don't actually see her go back for the knife but it's implied.

I only know all this becuase i had the same question and closely rewatched Grad Day p2 to figure it out.

As for the 'Choices' part, you'd naturally assume that Buffy would have just handed the knife back to Faith. Faith obviously would have wanted it back, being a valued gift from the Mayor and all. I'm all blurry on all the details of how that played out now.
