Only took 7 tries.

When Mal is talking to Simon in the passageway right outside the engine room at the beginning of the movie he says he needs to be "innoc'ed" (inoculated) before going planetside. Hence the injection Simon administers as they argue about River's participating in the upcoming event.

I've seen the movie 15 times and I had to read the Visual Companion before I got that one. Every time I saw the scene I kept thinking that is such a weird thing to do... ooo.. maybe it's a CLUE!
