That's a tricky question. I always knew him as "Derrial" Book. Though his first name used to be "Meria". Joss changed the name, towards the end of shooting the BDM.

Hmmm - that's tricky, mainly because he specifically says 'on the crew' as if there is a distinction, in which case Simon and Book wouldn't count as crew.

Alternatively, you could also say that Inara punched Mal in The Train Job, although it was more of a slap.
