Wash didn't give it to her, he only helped her remember that she had one and he always will. Interspersed with the dream-bits are normal color, crew bits dealing with the way the developing political unrest post-Miranda affects the crew as a whole with everyone extremely insecure about the future of their lifestyle and no one more so than Mal. Cowardly Lion Mal tells them they are better of separate, even though it's not what he really wants, Scarecrow River hides out in the informary with Zoe (while narrating the dream bits) and has no easy fix ready, while Dorothy Inara watches news of rioting on Sihnon and contemplates which place is no place like home. We close on Zoe waking up with a new will to live and be happy, faced with the bad news of Mal's separation plans and everyone's uncertainty. She looks both resigned and determined. Like anything is possible now that Zoe's back in full force.

I always read it as Zoe refusing to truly compromise in their marriage. Wash has completely adapted to Zoe's life, but she's not willing to contemplate changing her life for their common goal of having children. She's hardly dealt with the war, has no life seperate of Mal's ideas - safe the choice to marry - and now she wants to add children to a dangerous environment. If Wash hadn't refused, I'd have declared them both insane, frankly. :)
