In the pre-release screening I saw on 5/5/05 it was very apparent that the sword went completely through Mal's side, then he pulled it out and continued fighting. It's at the point when the Operative asks him "Do you know what your sin is?" and Mal's reply, after pulling the sword out, "I'm a fan of all seven, but I'm gonna have to go with wrath." I'm pretty sure it is in the first dvd commentary that Joss said there were too many complaints that it seemed he was too graphic there, and that it was a wound Mal shouldn't have been able to shrug off so casually, so he edited the scene. You can still see Mal's action as he is pulling the sword out, but the actual impalement and removal is off-screen.
